We need volunteers for our MET program. Our riders are so grateful when they can get to their Dr.'s appointments.

As one of our riders said, ?It?s so good to come out of the doctor?s office and know someone will be waiting for me.?

The need for medical escort transportation is increasing every day. If you could offer just one day per month to drive people to their medical appointments, just download the forms below, fill them out and send them in to us.

These are the forms you will need to fill out to become an MET driver. We appreciate your completing all the forms as instructed.

You much complete a background check form. Here are the instructions for completing the form.

Instructions for Filling-out Background Check

Roswell Police Department is very strict on the application being filling out in its entirety and correctly or it will not be processed. All fields must be filled out.

Full Social Security Number must be given. This is how they process the application and look for any criminal records.

Under ?Check Any Of The Following That Apply? check ?An applicant for employment providing care to the elderly?

In the last field which ask who you are giving authorization to for someone to pick-up your application please write, Beatrice Lester.

Lastly sign and date.
